Chief Complaint: Abdominal pain for 3 days
HPI: 28 year old female with diffuse abdominal pain, which she describes as cramping, associated with diarrhea, progressively worsening over the last 24 hours. She denies BRBPR or melena. It is now more localized in LLQ and not relieved with tylenol or imodium. She has no associated fever or N/V. She has no appetite.
ROS: General: tired, no appetite
CVS: No chest pain
Resp: Denies SOB
GI: See HPI
GU: Denies dysuria, urgency, frequency
GYN: LMP 2 weeks ago, G0/P0, denies vaginal discharge
SH: Single, 1 partner
Works in food service
FH: Mother with Chrohn's
PMH: appendectomy age 8
frequent sinus infections
Exam: BP 120/60, pulse 100, resp 16, supine
BP 98/50, pulse 120, resp 22, standing
Appears tired, pale
HEENT: moist mucous membranes, throat normal
neck supple, no lymphadenopathy, thyroid normal size.
Heart: Nl s1, s2 No mrg
Lungs: Clear to auscultation
Abd: slightly distended, normal bowel sounds. TTP LLQ with
rebound
Rectal: Heme positive stool
Medical Decision Making:
LLQ Abdominal pain with peritoneal signs and guiac positive stool. Consult with Dr. Smith (General Surgeon), who will meet Pt. in ER. Pt. sent by ambulance to ER, due to PT's degree of pain and orthostasis. IV NS started in office.
What level of service would you assign this established patient and is the billing affected if the patient is admitted to surgical service? What if the patient is admitted to medicine and you are the admitting Doc?
